I have it as a handout in Word but couldn't figure out how to format it for wiki. I'll get round to it one day. I get them to do it "anonymously" (if a group) just to get a global vison of their needs as a class, and then go through it as a group discussion activity at the beginning of a course and then later on during the year, as it raises some interesting comments from them and helps 'em to be more aware of how they're learning, what they're learning/they've learnt and generally makes them more active - and exigent (lazy teachers/cowboy language schools beware) - learners. As a freelance teacher, I depend very much on my success stories! For the complete beginner level, I have an L1 version for the first time round, but the second one gets done in English.
